Regarding Identification Documents

For identity verification, we require the submission of one of the following four forms of identification and a photo of yourself: 

• Japanese My Number Card (photo on the back is not required) 
• Driver's license 
• Passport (containing name and date of birth) 
• Official documents issued by government offices (showing name, address, date of birth, with a photo attached by the issuing office) 
• For non-Japanese residents without Japanese nationality: documents issued by foreign governments or authorized international organizations equivalent to the identification documents above (must contain name, address, and date of birth) 

All documents must be valid on the submission date. 

MOL CART office may request additional documents to verify your residential address or other certificates as needed. 

Supplementary Documents List 

• The identification documents listed above 
• National or local tax receipts or tax payment certificates 
• Social insurance receipts 
• Public utility bills receipts (electricity, water, gas, landline telephone, NHK reception fees)*1 
• Documents issued by government offices (showing name and address)*2 
• For non-Japanese residents without Japanese nationality: documents issued by foreign governments or authorized international organizations equivalent to the above identification documents (must contain name, address, and date of birth) 

*1 Foreign public utility receipts are not accepted. 

*2 Japanese My Number notification cards are not accepted. 

Documents with expiration dates must be valid on the submission date. Documents without expiration dates must have been issued within the past 6 months.
